In den letzten Jahren tritt zunehmend die Bedeutung des Tumor-Mikromilieus für die Pathophysiologie der Chronisch Lymphatischen Leukämie (CLL) zutage. Bei dieser Tumorerkrankung sind zelluläre Interaktionen der malignen B-Zellen mit Immunzellen oder Stromazellen innerhalb der lymphatischen Organe von essentieller Bedeutung für die Proliferation und das Überleben der Tumorzellen, die sich im peripheren Blut ansonsten nahezu nicht teilen (siehe Abbildung 1).

Adhäsionsmoleküle und Chemokinrezeptoren sind zentrale Regulatoren der Interaktionen von Leukämiezellen innerhalb des Tumor-Mikromilieus und stellen somit mögliche zukünftige therapeutische Angriffspunkte in der Behandlung der CLL dar.
Die exakte Charakterisierung dieses Tumor-Mikromileus hinsichtlich ihrer Adhäsionsmoleküle (und hierbei vor allem Integrine), sowie Chemokinrezeptoren, die zusammen das Einwandern, Überleben und Vermehren der malignen Zellen koordinieren, ist daher eines der wichtigsten Ziele in unserer Arbeitsgruppe. In der CLL sind die adhäsiven und signalgebenden Eigenschaften der Integrine und Chemokin-Rezeptoren sowohl auf expressioneller als auch auf funktionaler Ebene gestört, woraus eine gestörte Migrationsfähigkeit der Krebszellen resultiert (Abbildung 2).

Unsere Gruppe untersucht die Chemokin-, Integrin- und CD44-vermittelte Interaktionen der malignen B-Zellen mit Blutgefäßzellen und Knochenmarks-Stromazellen mit Hilfe der Fluoreszenz-Videomikroskopie. Dabei verwenden wir spezielle Flusskammern, die den Blutstrom simulieren und uns erlauben, die Wirkung spezifischer Inhibitoren direkt an humanen Immun- oder Leukämiezellen während ihrer Interaktion mit humanen Gefäßzellen, oder im Gewebe zu testen (Abbildung 3 und Film). Des Weiteren untersuchen wir in diesem Zusammenhang die Beteiligung verschiedener intrazellulärer Moleküle, sowie Integrin-linked Kinase (ILK) und Rho GTPasen.
